Student Reflection on Medical Coding Assessment: Error Analysis and Procedural Improvement Strategies
Overview
This reflection paper analyzes errors made on a medical coding assessment, identifying incorrect CPT code assignments across multiple questions. The student reflects on the root causes of these errors and proposes a systematic procedural approach to improve future coding accuracy, emphasizing the importance of understanding case requirements and following established coding protocols.
What this reflection paper covers
- Identification of specific coding errors in questions 2-13
- Analysis of root causes including failure to follow coding procedures and cross-check terminology
- Proposed solution framework with steps for establishing coding goals and selecting appropriate languages
- Recommendations for optimization including editor selection and iterative improvement

Based on the test results, there are several questions I did not do correctly. There are numerous codes that I got wrong. The first wrong code is question 2, which was answered wrongly and never aligned with the correct answer. Question 3 was also answered wrongly, where a wrong CPT code was assigned to the case. Question 4 was wrong because I failed to crosscheck the terms in order to arrive at the correct answer. Question 5 and also Question 7 through 13, which never followed the correct procedure to develop the right codes for the cases. The whole assignment required coding solutions based on the provided cases. I answered most of the questions wrong because I failed to follow simple coding rules or procedures.
In order to avoid wrong answers in future, it is important to have a procedure to follow when answering the questions. There is a need to take time and understand each of the individual cases, ensuring that all are answered based on the requirements stipulated. There are different steps that can be applied to ensure that an individual writes the correct codes. The first step is establishing the goal of the codes, ensuring that the right language is chosen. The second is choosing the right language that will assist you attain the objectives you have identified. This is important because it will enable you to fulfill the objective of each code and have a correct procedure for answering each of the cases.
