Implementation and Assessment of a Peer-Led Mental Health Support Network in Urban Hospital Settings

Overview

This document outlines a structured assessment plan for implementing a peer-led mental health support network within an urban hospital setting. The plan establishes multi-level evaluation metrics across daily, weekly, monthly, quarterly, and yearly timeframes to measure program effectiveness through patient enrollment, readmission rates, satisfaction levels, and staff integration, with specific key performance indicators including 25% reduction in 30-day readmissions and 80% patient-specialist connection rates.

Week 12 Leading Innovation Assessment Plan and Peer Critique Discussion

Assessment Plan

The main goal of this project is to successfully implement a structured Peer-Led Mental Health Support Network in an urban hospital to improve patient recovery, reduce readmissions, and increase patient engagement through non-clinical peer support.

The end assessments will measure the number of patients enrolled in the peer support network, the frequency of peer-patient contacts, and the reduction in hospital readmissions for mental health-related issues within 30 days. Additional data will track patient satisfaction and perceived support levels.

Indirect and secondary goals include identifying barriers to participation, improving staff acceptance of peer roles, and measuring overall program sustainability.

Daily Assessment

The peer champion and implementation lead will hold brief daily huddles with peer specialists and the mental health team to review new referrals, active cases, and any immediate barriers. Peer specialists will log daily interactions (number of sessions, type of support provided) in a secure system.

Weekly Assessment

The implementation lead will review weekly reports on peer contacts, patient attendance at support groups, and no-show rates. The mental health nursing manager and peer champion will meet weekly to discuss challenges, staff feedback, and adjustments needed for better integration (Gillard et al., 2024).

Monthly Assessment

The Director of Mental Health Services will receive a monthly report including enrolment numbers, patient satisfaction survey results, and readmission rates for enrolled patients. The multidisciplinary team will meet to review progress, identify recurring barriers (such as scheduling or stigma), and plan corrective actions.
