Ethical Decision-Making in Pediatric Gynecological Care: Balancing Parental Authority with Minor Autonomy

Overview

This case study examines an ethical dilemma in pediatric nursing where a mother requests pelvic examinations for her minor daughters. The paper analyzes the tension between parental authority and patient autonomy, proposing a framework that prioritizes informed consent, individual patient interviews, and confidential care while maintaining ethical obligations to both minors and guardians.

Case Study One

The first case study places a nurse in a compromising situation by presenting patients whose decisions seem to get influenced by the mother's health opinion. The first move will be to inquire from the parent about the essentialness of the examination she needs for her daughters. Family history factors or early childhood injuries could be some issues that propel the mother towards having a pelvic exam on her children. A family history of cysts, endometriosis, painful cramps, and heavy bleeding could trigger a pelvic exam on the girls. However, physicians handling minors have an ethical obligation of promoting the client's autonomy in treatment. Children, regardless of being accompanied by a parent in a medical facility, get expected to make an individual health decision. The decision-making ability of a minor relies on his or her age, emotional maturity, and knowledge regarding personal health (AMA, 2020). As a doctor, we have an ethical duty towards the patients and their parents or guardians.

Since the two girls are the patients, the mother will get required to grant them privacy with the doctor. The girls will each get separated for individual questioning. Before personal interviews, each girl will get informed that a pelvic exam is a means of a doctor to establish any illness in specific organs of a woman's body (Women's Health, 2020). Each of the patients will have a series of questions whose responses will get confined to them.
