Clinical Neurological Assessment of a 69-Year-Old Male Patient with Suspected Dementia

Overview

This case study documents a comprehensive neurological assessment of a 69-year-old African-American male presenting with confusion, memory loss, and poor coordination. The assessment integrates demographic, objective, and subjective data to arrive at a suspected dementia diagnosis and proposes a nursing care plan focused on patient reorientation, caregiver support, and discharge planning.

Demographic Information

The patient I encountered was an African-American male aged 69 years. Before undertaking the nursing assessment, the patient exhibited confusion and lack of focus. He was unable to tell where he was located. Further, the patient was also very reactive to light.

Objective Data

I found that the patient had regular sitting and standing balance. He also had poor coordination during movement. The man also exhibited memory problems, especially remembering recent events. He had reduced concentration, increased confusion, and behavioral changes. With the passing of each day, the patient was unable to do day-to-day tasks (Jarvis, 2011).

Subjective Data

I embarked on asking the patient some questions. I realized that there was clear inconsistency in his responses. This affirmed possible loss of memory and confusion. However, he was able to explain his physical state. He confirmed that he was unable to withstand intense light. The patient also experienced headaches and lacked sleep. There were no other severe medical conditions that affected the patient. He was placed under medication for pain relief. There was general discomfort and uneasiness after admission to the hospital (Jarvis, 2011).

Health History

The patient had memory problems because it was difficult to recall his medical history. However, his son, Brian, assisted in explaining the prior health status and how the current symptoms developed. It was established that Henry was diagnosed with diabetes and high blood pressure two years ago. There was a history of neurological problems within the family.
