Final Exam Performance Improvement

As a consultant assessing a hospital or nursing home’s Quality Improvement (QI) Program, I would follow a structured, evidence-based approach grounded in the principles taught throughout this course. First, I would review the organization’s QI plan and infrastructure, verifying leadership commitment, existence of a patient safety officer, just culture reporting systems, and use of an improvement framework such as Plan-Do-Study-Act (PDSA/PDCA) or Lean. Next, I would evaluate whether processes are repeatable, measurable, and standardized, examining key process measures (such as pressure ulcer risk assessments, medication reconciliation rates) rather than relying solely on outcome measures.

I would then assess the culture of safety through staff interviews, safety attitude surveys, and observation of huddles, walkarounds, and error reporting practices to confirm that frontline microsystems are empowered and non-punitive reporting is genuine. Data analytics capability, TeamSTEPPS training, root cause analysis proficiency, and stakeholder engagement (including patients and families) would be audited next. Regulatory readiness would be reviewed by mock-tracing National Patient Safety Goals, infection prevention, and medication management standards from The Joint Commission and CMS. Finally, I would benchmark performance against national databases, review sustainability of past improvements, and ensure alignment with current initiatives such as CMS Star Ratings and preventive health strategies.

This course deepened my appreciation for continuous, incremental improvement over corrective approaches. I most enjoyed the real-world application of PDCA to personal and professional settings.
