A prospective cohort study was conducted to investigate the incidence of heart failure (HF) among those with coronary artery disease (CAD). 175 participants with CAD yet free from HF enrolled in this study. They were then followed up for 4 years. Below you can see the results:

• Year 1: 15 cases of HF were detected; 14 lost to follow-up; 7 fatalities

• Year 2: 27 new cases of HF detected; 9 lost to follow-up; 7 fatalities

• Year 3: 19 new cases of HF detected; 10 more lost to follow-up; 15 fatalities

• Year 4: 32 new cases of HF detected; 17 more lost to follow-up; 24 fatalities

Given the above info, determine the incidence rate of HF in this study; show your work (5 pts). NB: Make the assumption that subjects with new HF diagnoses and those lost to follow-up were free from HF for half the year, and thus contribute ½ year to the denominator. Please HELP!
