Project Aim: To evaluate the impact of introducing the Intervention to Reduce Acute Care Transfers- Situation, Background, Assessment, Recommendation (INTERACT-SBAR) tool on the rehospitalization rate for post-acute patients residing in a skilled nursing facility (SNF), compared the non-standardized telephone reporting by licensed practical nurse/registered nurse (LPN/RN) staff to providers within an 8-week period.
